Cerfolio, 34, will move up from assistant GM to serve under president of baseball operations Chaim Bloom.
"Since joining the Cardinals two years ago, Cerf has spearheaded tremendous progress in our player development efforts from top to bottom, and in the advancement of our performance group and its integration within our entire organization," Bloom said in a statement. "He is relentless in his desire to move us forward, and is invested in relationships with both new hires and longtime staff alike."
St. Louis hired Cerfolio away from the Cleveland Guardians in October 2024.
Cerfolio played college baseball at Yale University.
The Cardinals went 77-85 in 2026 and missed the playoffs for the fourth straight year.
